PARENT FORMS
The following are forms that
parents must fill out
before their child (children) can attend Camp Lookout.
HEALTH HISTORY/PERMISSION FORM
If you need a copy of any of the forms, simply click on the name above and print a copy. All of these forms should be brought to registration on Sunday.
The financial statement is filled out once per summer per family. The health history and medication formsneed to be signed by your health care provider, if possible.
APPLICATIONS
The following are application forms for Camp Lookout.
(Non-counselors applying for other staff positions only need to complete pages 1 and 3)

Please note:
The original application
for a child to attend camp is NOT
available on line. The registration form is available through the school nurses, the Department of Social Services, or by contacting the camp.
